How can I help my patients to keep active?
There are a number of online resources that you can use to guide your patients to keep active at home, and to restart their exercise program should they have stopped. These tools and resources are coming from the Canadian Pulmonary Rehabilitation Program. This new program supports healthcare professionals with high-quality content and guidance for delivery; the full program can be found on the LivingWellwithCOPD.com website. You need to create an account as a healthcare professional and once you login, go to the “Rehabilitation” tab.
Self-management education and behavior-modification components included in the CPRP program:
- Standardized learning materials for patients
- Reference guides for the healthcare professional describing how the intervention needs to be done (education topics, exercise program)
- Presentations that can be delivered to patient(s) in-person or online
- A clearly defined plan from the initial evaluation to the maintenance phase
